My employer switched us to Caremark as of January 1, 2011. Since that time it has been a constant battle with Caremark to get my prescription for Naratriptan filled. I sincerely believe that they are giving me all this grief only because this drug is expensive.

I got a new prescription from my Doctor in February. "Take one tablet at onset of migraine, not to exceed 2 tablets in 24 hours". Caremark refused to fill this because the prior carrier (Aetna)filled the prescription in December, and Caremark will only authorize 27 tablets every 90 days. Never mind that it was a new prescription from the doctor, never mind that Caremark has NEVER filled this prescription before. So I waited and paid out of pocket (ouch I pay handsomely for medical coverage through my employer). Finally I was authorized by Caremark to get 9 pills on March 26. But the pharmacy messed up and only gave my 4. So I called today to ask about the remaining 5 pills. Caremark can't handle this. They want the pharmacy to reverse the original fill and redo it for 9 instead of 4. Whay can't Caremark just authorized the fill for the 5 remaining pills? Even better, why don't they accept the doctor's prescription for 27 pills, refills as needed, no more than 2 a day?

1)"There is always a Qty limit" - Not true. I have been taking this drug (Amerge/Naratriptan) for 6-7 years. When I had Kaiser through a prior employer, there were no limits and I paid $10 for 27 pills. Up until January of this year, my employer offered drug coverage through Aetna, and I was never restricted on refills. What was especially annoying about Caremark, is that Aetna paid for the fill in December 2010, and Caremark refused the refill based on Aetna's fill date. 2)I was able to get the remaining 5 pills of my 9 pill refill by returning to the pharmacy, where they had to call Caremark and void(?) the first fill and then give me 5 more. Huge hassle for all involved. Yes, shame on me for not counting my pills before I left the counter. I am sure everyone in line behind me would have understood the delay.
I recently jumped through Caremark's hoops and had the Dr. do a prior authorization for the Naratriptan. So now I can get 18 pills every 25 days instead of 9. How lucky can I get? I remain extremenly disatisfied with this plan and would never choose it if my employer gave us other options.

2)I was eventually able to get the remaining 5 pills of the 9 pill refill after the pharm tech called Caremark and voided(?) the original quantity. Big hassle for all involved. Shame on me for not counting the pills before I left the counter. I am sure everyone waiting behind me would have understood.
I remain extremely dissatisfied with this plan and would not choose it if my employer offered another option.
